Optimizing pediatric follow-ups with the electronic transmission of ABCdaire questionnaires

To improve the efficiency of pediatric follow-up visits, the "Child monitoring (ABCdaire)" questionnaires can now be sent to parents electronically.
This approach allows parents to complete the forms ahead of time, at their own pace, giving them the opportunity to reflect on their answers, prepare their questions and concerns, and ultimately make the consultation more efficient.

Step 1: Define the timing and internal process for sending forms

Since the electronic sending of forms is done manually, it is essential to determine when and by whom the forms will be sent (e.g., when scheduling the appointment, a few days in advance, or upon the family's arrival in the waiting room).

It is recommended not to send the forms too early to prevent the email from getting lost and to ensure the information provided is up to date.

ℹ️ Upcoming enhancements
In the coming months, new filters will be added to the Appointments module. These filters will allow schedules to be sorted by activity, making it easier to send the questionnaires (e.g., sending them one week in advance for upcoming follow-ups).

In the longer term, the team aims to introduce a feature that will allow automated form transmission based on the appointment activity.

Step 2: Send the questionnaire to the family

ℹ️ In order to send forms electronically to your patients, an administrator must first activate the clinic’s patient portal.

To send a form, follow the steps outlined in this article.

💡 Tips

  • Contact details: Make sure the email address and mobile number are correct. By default, the system uses the first email address and mobile number listed in the patient’s administrative profile.

  • Joint appointments: If multiple professionals are involved in the appointment, it’s best to leave the Requester field empty when sending the form. This allows all clinic users to see the notification that a form can be imported into the patient’s chart. If a requester is selected, only that user (or someone who has selected their mandate) will be able to view and import the form.

  • Prepare the family: Add a note to the appointment activity to inform the family that an electronic form will be sent and should be completed prior to the visit. This note will automatically appear in booking confirmations and appointment reminders (if sent via Omnimed).

Please note

  • Instructions are included for the family, indicating that their answers are confidential and will be discussed during the consultation. They are also advised that they can skip any unclear questions and ignore the codes in parentheses (e.g., A*, 2A, B, etc.).
  • Questions have not been reworded for patients and appear as-is. However, only the following sections are visible to families: Parental concerns, Life context, Health monitoring and recent evolution, Current life habits, and Developmental monitoring. Any instructions or information intended for professionals are not shared with the family.

Step 3: Import and review the forms completed by the parent

Once the form is completed, a black banner will appear at the top of the patient's chart:

  • for the designated requester;

  • for any user who has selected their mandate;

  • or for all users if the Requester field was left empty during the form submission.

The form can then be imported and reviewed with the family. You can refer to this article for the form import process.

Please note:

  • Answers can be edited after import. The note’s history will retain the original version of the form as it was imported.

  • Previous answers are not retained in the patient’s form interface. This means parents will not see any previously entered responses, even if the same questions were answered during earlier visits. Empty fields will need to be completed again in future follow-ups.
