Care Trajectory - Chronic Obstructive Pulmonary Disease (COPD)

Omnimed is a pioneer in developing medical projects that fit into the reflexive practice* process promoted by the Collège des médecins du Québec. These projects aim to improve the management of potential patients or who have been diagnosed in a targeted therapeutic area.

The COPD project is part of the reflexive practice and is aimed primarily at first-line professionals, in collaboration with medical specialists. This project aims to better target the COPD clientele, or potentially COPD. The tools developed to contribute to optimizing patient care trajectories and adjusting their treatment according to GOLD (global initiative for chronic obstructive lung disease) standards.

The creation of clinical tools, reports, and dashboards integrated into the EMR allow family physicians and professionals such as nurses, respiratory therapists, and pharmacists to more effectively identify their COPD and potentially COPD patients. The identification of critical criteria or "quick wins", integrated into the reports, makes it possible to target the patients more at risk. Professionals can, therefore, identify their potential patients, establish a diagnosis, and adjust the therapy, using the tools available. In addition, partnerships with pulmonologists allow electronic consultation in more complex cases, for example.

Pilots

Five pilot clinics, located in Estrie and in the Quebec City area, have been identified to date for the implementation of this project. This project is under development and expansion discussions are currently underway.

* Reflexive practice allows physicians to question their practice, solve complex cases and implement new ways of doing things to improve patient care.
