Modify the patient's personal information (name, HIN, gender, birth date, deceased date)



The Administrative center allows you to modify the name, the HIN, the date of birth, and the sex of your patients. Before modifying a piece of information, make sure that there is no duplicate record for the patient.

⚠️ If there is a record duplicate, it is impossible to save the modification, see the article Patient records fusion explained for the steps to follow to merge patient records.

Modify the patient's personal information

  1. Access the patient Administrative center, whether it's by being redirected when you create a new patient or by clicking on the patient icon beside the name of the patient located at many places throughout the application.
  2. Click on Modify in the Personal information section.
    ⚠️ Orange warning messages will be displayed under the fields with potential error. Please note that a validation is made on the NAM to see if it fits with the first and last name, the date of birth and the sex of the patient, as in Québec, these information need to fit. As we allow you to add other insurance numbers like the HIN for Ontario, you'll be able to save the information even if there are still warning messages. It is then really important that you make sure that the information you save is accurate.
  3. Click on the information you wish to change.
  4. Click on the Save button.

🔒 Synchronization with the QHR

When the patient's record is synchronized with the QHR, it is impossible to change the patient's first name, last name, NAM, gender, and date of birth. The fields are then greyed out and a message indicating the synchronization is displayed at the bottom of the file. Contact us via our chat to receive help.

Update to QHR

In order to change this information to the QHR, you can provide the patient with these instructions: Right to have your health information corrected in the QHR so that he or she can send the duly completed request to rectify information form to the person in charge of access to information at the Ministère de la Santé et des Services sociaux.

Update in Omnimed


It is also possible to make the change in Omnimed. To do so, contact the person with administrative rights in your establishment so that he can make the change from his Omnimed account.
